FL-DDDRP PROGRAM (registration only)

Diapers will be distributed only to individuals who have registered for this event and meet all requirements.

Applicants must complete the registration form below (or click “RSVP”) and upload the following documents:

• the child’s birth certificate

• proof of enrollment in a daycare program.

Eligible children must be between 0 and 3 years old and currently enrolled in daycare.

Pickup details will be sent via email after registration is reviewed and approved.
